How to Create a Form Template

Forms in Engage can be used in a wide variety of ways. If you are unsure where to start or what forms to build, click here for a variety of suggestions and ideas. 

To create your form template, click on Forms / Templates / "New Template". Please note that only those with Forms: Manage, and Admin: Companies authorities have the ability to create new forms, and view form templates. 



When building your form, there are a variety of different elements on the side that allow you to completely customise, as well as link to other areas of Engage. You can add these by either clicking on them to get them to appear at the bottom of your template, or drag-and-drop to move them around on your form.

Once you have completed your Template, you have the ability to Schedule the form so that specific employees have access, as well as set automated alerts and actions, or scores and colours behind the scenes, which are triggered when a specific answer on your form is selected by the employee filling it out. 

You can also Copy an existing form template (Forms / Templates / "Copy" next to the form you would like to copy) if you would like to make some alterations. Please note that you are unable to edit a form if it has already been assigned to an employee. 



Making a Form Inactive

If your form is no longer required, or you wanted to update a form with a new version, you are able to make the old form Inactive to remove the form template from an employee's form template listing, and they will no longer be able to complete that version. 

Please note if you make a form Inactive, the history of all completed forms is retained in Engage. If you wanted to refer back to an old template, or Copy an Inactive version, you can do so by clicking on 'Show Inactive Forms' to get them to appear in your template list.
