The Forms module is an additional module in Engage. If you do not currently have the forms module switched on and would like to learn more, please email support@engagesolutions.co.nz for more information.
Please note, only those that have the Forms: Manage, or Admin: Companies authorities will be able to pull reports on completed forms. To learn more about authorities, please click

To pull a report, click on Forms / Reports via the blue side menu. From here, you will see a list of all of the assigned forms. Note that if a form has not been assigned, it will not appear here until it has been assigned. To learn more about how to assign forms, click

If you click on the specific template under Active Templates that you want to pull a report from, all of the raw data from the form answers will then be accessible in a table grid format, including a copy of the PDF under the Download Form column.
You are able to use the Filters function to specify required information, such as forms completed within a specific time period, and you can extract to Excel / CSV etc as needed via the buttons next to the Search.
If you wanted to pull a report on a previous Inactive template (that is no longer in use), you will find this by clicking on the Inactive Templates blue button, which will expand and show you any inactive form templates.
